Application deadline is January 8, 2010. (If mailed, it must be postmarked by January 8, 2010.)

Application fee is $90 per player and proceeds benefit the Boys & Girls Clubs of Greater Fort Worth.

Field is limited to 72 contestants.

Applications will not be accepted without payment (check, money order, Mastercard, Visa, American Express).

All payments will be returned if the applicant is not accepted.
